ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-add <br />INSPECTION DATE: l2/19im INSPECTORoS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2007-2008. Present during the inspection was <br />Mr. Joe Lamanna quarry manager, Mr. Joel Bolduc Environmental Manager & Mr. Thomas <br />Newman, Chief geologist. Site is a large 112 Lime Stone and sand and gravel quarry, <br />covering 1600 acres of permit and affected area with post mining land use as Range <br />Lnad.. <br />Prior to the physical inspection, the Division sat down with the representatives and <br />explained the few issues that still remain to be resolved after Technical Revision #6 <br />and Amendment # 1 were approved in 2003. Mainly the fact that the Division was going to <br />recalculate the financial warranty for the site and also set ground water monitoring <br />compliance points with numeric protection levels. Due to specialist resigning and <br />retiring both issues were never completely resolved. It is the Division's intention to <br />resolve and complete all the remaining unresolved issues with in the next 6 months. <br />Also discussed the possibility of regulating the Kiln Cement Dust disposal area <br />falling under the jurisdiction of the Colorado Department of Health and Environment, <br />Hazardous Waste Section and encouraged the operator's to see if such disposal is in <br />deed covered under their jurisdiction. At present DRMS, had accepted the responsibility <br />and had authorized the material(CKD) to be used as backfill in the limestone quarry. <br />The Division requested Hocium, commence with their Gw monitoring plan on a quarterly <br />basis, starting with winter quarter and once the Division receives a couple of quarters <br />worth of data, it will be compared with the existing five quarters of data and THE new <br />TCLP, data for the cement Kiln Dust. If the new data, still does not shows no toxic <br />metals to be present, then DBMS will set numeric protection levels for a few selected <br />wells for the site, such as Selenium, Sulfate and Manganese. Operator agreed to <br />commence the ground water monitoring and also provide one set of TCLP data from the <br />cement Kiln dust. <br />1) After the meeting, began the physical site inspection by getting an overview of the <br />affected areas from the top of the clinker plant. It was active at the time of the <br />inspection. From the top looked at the Overview of the affected areas, including the <br />two pits, CKD disposal area and all the facility structures. In addition along the <br />East and South side of the affected area small stockpile of numerous sizes of old <br />tires were noted. The operator's explained that these tires were being utilized in <br />addition to coal, as fuel for the clinker production. Since old tire placement was <br />never included in the approved permit, operator was notified to include these area <br />in the Technical Revision, that will be submitted for the upland diversion that the <br />operator propose to put in place along the North West side of the limestone quarry. <br />2) After the overview from the top, continued the inspection with Joe Lamnnna. None of <br />the facility buildings, clinker silos,trummles, office buildings, tire shredding <br />facility, and all support buildings for the production area were ever included in <br />the previous warranty, which stands at present at $268,550.Since all the facilities <br />have to be demolished and removed from site, the Division request Holcim to provide <br />details of all structures in the area, with a very detailed faculties map showing <br />their locations, square footage, and kinds of material they were constructed from. <br />In addition thickness of all concrete footings, for all the structures including the <br />office buildings and shops. <br />3) Inspected the pit areas, both were active at the time of the inspection. Observed <br />the are where cement kiln dust was being used as backfill. According to Holcium, <br />cement kiln dust at the plant is produced at smaller amounts than was anticipated <br />when TR #6 was submitted and approved. <br />4) Observed the monitoring well. As reported above, the Division has requested the <br />operator to start sampling all monitoring wells and conduct one TCLP test on the <br />cement kiln dust and provide the results. DRMS will evaluate the data and compare it <br />with results submitted in 2003. If the changes noted are negligible, after the data <br />is correlated with existing results, DRMS will set numeric protection levels for <br />those metals that are above the classified protection levels for the GW regime and <br />
