Whirlwind Mine Page 5 of 8 <br />MC /SPCC 06/29/07 <br />Section 3.0 -Government Agency Notification <br />The Technical Services Supervisor or Corporate Environmental Manager will determine if <br />any release of hazardous material has occurred in Reportable Quantities (RQ). <br />1. The release of an RQ hazardous material will then be reported within 24-hours of the <br />occurrence in the following manner: <br />I ,1.Hazardous Materials RQ spills will be reported by telephone to: <br />The State of Colorado Emergency Management Unit (EMU): <br />24-Hour Emergency Number: 1-877-518-5608 <br />if unable to reach the EMU, then - <br />US Environmental Protection Agency <br />Region VIII Office <br />Denver, Colorado <br />USEPA 24 Hour Emergency Spili Number: 1-800-424-8802 <br />If the spill enters surface waters where the possibility of harm to fish or aquatic life exists, <br />or the spill enters land where there is a possibility of harm to game or wildlife habitat, <br />EFR will also notify the State Division of Wildlife by telephone: <br />Colorado Division of Wildlife <br />NW Region Service Center <br />(970) 255-6100 <br />1.2.Fue1 and Oil spills will be evaluated to determine the urgency of the reporting <br />requirement. <br />1.2.1. Spills that enter a water body will be reported immediately #o: <br />The State of Colorado Emergency Management Unit (EMU}: <br />24-Hour Emergency Number: 1-877-518-5608 <br />if unable to reach the EMU, then - <br />USEPA 24 Hour Emergency Spill Number: 1-800-424-8802 <br />1.2.2. Spills greater than 2S gallons that are contained and will not reach a water body <br />will be reported following the written report criteria listed in 1.3 below. <br />1.3.Following telephone notification, EFR will submit a written report to the appropriate <br />agencies or officials within five working days after the occurrence. <br />
