Rule 4.05.13(4)(c): A hydrologic report shall be submitted to the Division annually with the date
of submittal determined in consultation with the permittee. The annual hydrologic report shall
contain at a minimum:
The report is in compliance with this rule. The report is to be submitted to the Division by April
1 following closure of the report year (Permit, page 4.05-11). The report was received in the
Division on March 25, 2004. The missing reports were transmitted by a letter dated Apri129,
2004, and received at the Division on May 5, 2004. Water monitoring requirements are identified
on pages 4.05-1 I through 4.05-14 of the permit. There are two surface water-monitoring stations
