1) The location and number of acres in the various stages of reclama-
tion (oackfilled, graded, topsoil replaced, re vegetated),
2) The species of vegetation planted, the locations and approximate
dates of planting,
3) The type and approximate quantity of fertilizers, organic material
or soil contlitioners used and location of use,
4) An appraisal of the success of reclamation efforts, including
locations and number of acres reclaimed for that year, and in the event of
failure of any efforts, a short narrative on the suspected or determined
causes of sucn failure, and
5) The affective date of the information provioetl above shall oe no
more than 45 days prior to the anniversary date.
