Buckner Owner-Operator (Owr. - Opr <br />MINING - RECLAMATION PERMIT No. M-93-030 <br />As required and directed, as (Owr. -Opr.) I am submitting a <br />report, an updated "Affected Lands Map" (ALM), and the completed <br />report sheet sent to me in February 1997 to provide information <br />regarding mining-reclamation operations that were conducted during <br />1996 on the areas subject under Permit No. M-93-030. Also please <br />find my personal check for $75.00 as payment for the required <br />annual fee. <br />NOTE: <br />The (ALM) has been provided with a supplemental LEGEND item which <br />explains a color code that has been utilized at the several <br />locations on the (ALM) to indicate the extent of any mining-recla- <br />mation work conducted during 1996 from April 15th through Nov. 30th. <br />I am of the opinion that the color code is satisfactorily self <br />explanatory and should suffice to assist anyone examining the map <br />to determine the type of operations that occurred at any particular <br />site. If no color other than blue or white appears at a site, then, <br />nothing of significance has been done at that site. <br />The following report is submitted with the understanding that <br />simultaneous comparing of the (ALM) will be required to fully <br />comprehend items treated in the report narrative. <br />Sincerely, <br />Owner-Operator Crystaljack Mining <br />John 0.
