ATTACHMENT TO ANNUAL REPORT <br />MONOGRAM-JO DANDY MINES RECEIVED <br />PERMIT NO. M-1977-293 <br />NOVEMBER 27, 2002 OCT Z 9 2002 <br />Division of Minerals and Geology <br />The installation of a Permit identification sign, as stipulated as a Corrective Action item following <br />the June 4, 2001 inspection, has been partially resolved (see discussion below). <br />Another site inspection was conducted by the Division of Minerals & Geology on July 17-18, 2002. <br />The Monogram-Jo Dandy Permit includes a number of small isolated old mines, and considerable <br />effort in the field and detail examination of old records has been conducted to determine which sites <br />are specifically covered under the Monogram-Jo Dandy Permit, and to determine the necessary <br />follow up actions. The following Corrective Actions have been stipulated after the July 17-18 <br />inspections: <br />- A reclamation schedule was required to be submitted by October 11, 2002. This Action <br />was completed. <br />- Although a permit identification sign has been installed, due the scattered nature of the <br />individual mines under this Permit, individual mine identification signs are needed at the <br />Anna May mine and the Great Hesper pit. The installation of the signs is required by <br />October 31, 2002. <br />- A number of the sites have been previously reclaimed or partially reclaimed; the <br />Division has requested that final reclamation be conducted this fall if the partially <br />reclaimed sites are not to be activated in the future. Final reclamation work has been <br />underway since the second week in October, although the unusually wet October <br />weather is hampering completion of site work. <br />The following reclamation work is completed or currently in progress: <br />- Great Hesper pit -minor regrading to control rilling and erosion; re-plugging portals <br />where slumping has occurred; trash removal and general site cleanup; re-seeding <br />previously seeded areas where vegetation is sparse. <br />1 <br />
