NE1/4 Q7'R QTR, SEC. 26 T. 1S R. 98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACII.ITY NUMBER: Plant tank farm area OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-Ol 18327 UMT NAME/CA NUMBER: None <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />OiUSaltwater Spill OiVToxic Fluid Spill <br />Other (specify): Process water from draining product pipeline from wells <br />CAUSE OF EVENT: Flueeine problem in new tube heat exchanger , required drainine product line onto <br />concrete pad west of tank farm. A drain valve in the tank farm was inoperable. and as a result sample lines on <br />small pad were used to drain lines. Sprav and pad leakage were a result of large volumes drained on the pad. <br />VOLUMES OF POLLUTANTS: Discharged/consumed: Aooroximately 500 gallons <br />Recovered: None <br />TIME REQUIRED TO CONTROL EVENT (in hours) 2 hours <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: Water on the pad 'was drained to tank farm within an hour of event. Bicarb contaminated <br />gavel and soil removed to disposal pit on 6-2-98.
