I T. 2S R. 98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACILITY NUMBER: 1 A-SHR OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-O118327 UNIT NAME/CA NUMBER: None <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />OiVSaltwater Spill Oil/Toxic Fluid Spill <br />Other (specify): Sodium bicarbonate production fluid <br />CAUSE OF EVENT: Flex hose and wash water line to 1 A-SHIT well head leaked. Prior to the leakase. <br />excessive line pressure was noted on the pipeline, as a result of plugQintiproblems in heat exchaneer at the <br />plant. The hi¢h line pressure caused pipeline to fall off pipe supports (5-20-98), and may have created <br />mechanical failure of the wash water line. The flex hose was replaced 5-2-98. but replacement hose failed 5- <br />21-98 probably due to high line pressure <br />VOLUMES OF POLLUTANTS. Discharged/consumed: Approximately 1500 gallons <br />Recovered: None <br />TIIvIE REQUIItED TO CONTROL EVENT (in hours) I hours <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: Recovery well 1 A-SHIt pump was turned off, and flow to injection well 1 A-4HI was stopped. <br />Approximately 10% of the spill flowed to 1 A-SHIt mud ~t before drainage pipe from well head plugged due to <br />bicarb solids. The remainder of the spill flowed out onto the surface of the pad, but was contained on the drill <br />pad. The flex hose was replaced and wash water line repaired on 5-21-93. Also on 5-21-98 a ditch sufficient <br />to drain well head leaks was consu~cted to the mud pit.
