Logan Wash Mine Annual Report <br />Permit No.: M-7977-424 <br />Well LW-22 is located adjacent to Well LW-242 in SEl/4 SEl/4 SW1/4, 527, T7S, R97W. The <br />well is 83.6 ft deep, depth to water was measured to be 7.8 ft, and the well has a 5-inch inside <br />diameter casing. The well was sampled on November 10'h, 2005 at the request of Mr. Prather. <br />Mr. Prather is planning on using water from the well upon establishing an agreement with the <br />BLM. Well LW-32 is located in the NE1/4 SW 1/4, 524, T7S, R97W. The depth to water was <br />measured to be 253. 65 fr. The depth of this well is greater than 1000 ft. Well LW-45 is located <br />in NEl/4 NEl/4, 519, T7S, R96W. The depth to water was measured at 247.6 ft in the annulus <br />space. The annulus total depth is approximately 595 fr, and the inside diameter is 7 inches. A 2 <br />3/8-inch tubing inside of the 7-inch casing is obstructed at a depth of 8.65 ft. Well LW-1 ] 6 is <br />located in the NW 1/4 NW ]/4, S20 T7S, R96W; no data is available for this well. Well LWCW- <br />lA is located in the SEl/4 NW 1/4, S25, T7S, R97W. This well was drilled in 1999 as a down- <br />gradient compliance well for post-closure monitoring purposes. <br />4.9 Removal of Reclamation Fence Materials <br />Reclamation fencing originally installed to protect previously revegetated areas along the Logan <br />Wash access road was removed and stockpiled in 2004. The stockpiled material was removed <br />during this reporting period. Fencing consisted of barbed wire, metal T-posts, and wooden fence <br />posts. Areas where fencing was removed include Area 6, 20, B2-B5, as shown in Exhibit F in the <br />mine permit. All areas where fencing was removed contained well-established native vegetation. <br />4.10 Re-grading <br />Re-grading with conventional excavation equipment consisted of grading on previously disturbed <br />areas to construct the rip-rap and non rip-rap storm water drainage channels on the Research <br />Mine and Lower Bench, and other drainage channels such as toe drains and v-ditches along the <br />Upper and Lower Bench roads. Re-grading was also conducted at the new Soil Barrow Area <br />prior to revegetation to contour and modify the slope to a grade less than or equal to 3 fr <br />(horizontal) to 1 ft (vertical). <br />4.11 Release of Warranty for Reclaimed Land <br />On August 29°i, 2005, DMG conducted a site inspection of lands meeting reclamation <br />requirements. The visit was originally requested by OOSI in January of 2005 and was revised in <br />a letter of January 4°i, 2006. On January ] 2`h, 2006, DMG released OOSI from further <br />reclamation responsibility for Areas 6, 8, 9, l2, 14, 15, 16, 17, 19, 20 and a portion of Area 13 <br />(roads). <br />5.0 Maintenance Activities <br />Maintenance activities included periodic inspections of mine roads, portals, mine water drainage <br />systems associated with the retorts and general mine workings, as well as inspection of general <br />evaporation pond operation, security, and leak detection system. Maintenance activities <br />conducted at the mine and Pond 1 are summarized in the following sections. <br />5.1 Mine Maintenance <br />During the reporting period and while site reclamation work was being conducted, GSHI <br />inspected the Logan Wash Mine site on a weekly basis during April through December. <br />]nspection of the mine site was typically restricted to the surface azeas of the mine benches, roads, <br />Western Water 8 Land, Inc. 8 <br />
