Cattany <br />Division Director <br />Rule 4.05.9(17) All impoundments shall be examined at least quarterly, by a qualified person <br />designated by the operator, for appearance of structural weakness and other hazardous conditions. <br />The reports shall be retained at or near the mine site, and a copy of the report shall be submitted to <br />the Division on a quarterly basis, or other approved submittal frequency. <br />The report is in compliance with this rule. The reported dates of inspections for appearances of <br />structural weakness and other hazardous conditions are presented in the following table. A <br />qualified person made the inspections. Reports are maintained at the mine site. A copy of the <br />report(s) is submitted to the Division on an annual basis (by April 1 following) in the Annual <br />Hydrology Report (AHR) (Permit page 4.05-11). Colowyo was permitted to reduce the frequency <br />of inspection for stock ponds/small depressions on June 5, 2002. Inspections were conducted <br />during the 1 s` 2"a and then the 4`h quarter of the calendar year.
