Rule 4.05.9(17) All impoundments shall be examined at least quarterly, by a qualified person
designated by the operator, for appearance of structural weakness and other hazardous conditions.
The reports shall be retained at or near the mine site, and a copy of the report shall be submitted to
the Division on a quarterly basis, or other approved submittal frequency.
The report is in compliance with this rule. The reported dates of inspections for appearances of
structural weakness and other hazardous conditions are presented in the following table. A
qualified person made the inspections. Reports are maintained at the mine site. A copy of the
report(s) is submitted to the Division on an annual basis (by April 1 following) in the Annual
Hydrology Report (AHR) (Permit page 4.05-11). Colowyo was permitted to reduce the frequency
of inspection for stock ponds/small depressions on June 5, 2002. Inspections were conducted
during the 1 s` 2"a and then the 4`h quarter of the calendar year. Maintenance was accomplished as
indicated.
