• 3.1.3 Regulatorv Submittal/Approval <br />WRN submitted routine logs, surveys, Applications to Drill (APD's), Sundry Notices, <br />monthly, and quarterly reports to appropriate agencies. Temperature and gamma logs <br />were submitted for well 4A~A prior to injection and temperature logs were submitted after <br />injection to verify well mechanical integrity. No modification of the Mine Plan was required <br />for the BLM, EPA, or Division of Minerals and Geology (DMG) in 2000. Sundry notices <br />were submitted to the BLM and EPA for plugging and abandonment of well 90-5, and new <br />screens installed in production wells 4A~V and 3A-5V. No modification of the financial <br />bonding amount was required for the BLM or EPA in 2000. <br />The Colorado Department of Public Health and Environment (CDPHE) and Air Pollution <br />• Control Division (APCD) issued the final air pollution emission permit (98R60843) for <br />Sturtevant air classifier and baghouse. WRN submitted APEN renewal forms and fee for <br />86R6140-6 and 86RB140-7, the Fulton boilers and 86R6140-9, the dryers. <br />Two undesirable event reports were filed in 2000, three filed in 1999, and thirteen filed in <br />1998. Reports for process pond overflow to waste pond and a wellhead leak were <br />submitted to the BLM. No notices of noncompliance were issued in 2000. An inadequacy <br />notice was received from the DMG but withdrawn after WRN's annual report was located in <br />their office. As a result of an EPA inspection 5-22-00, a Class V injection well form was <br />submitted for the sanitary water system and to obtain rule authorization letter. Twelve site <br />• inspections by the BLM and one inspection by the EPA were performed. <br />7 <br />
