(BMRI) - San Luis <br />DMG Permit No. M-88-112 <br />Dear Mr. Dillie: <br />As you recall, BMRI contacted you on September 2, 1994, <br />concerning an inspection of the tailings facility raise <br />after an intense rainfall event that occurred during the <br />night of September 1, 1994. On September 3, 1994, Rick <br />Frechette from Steffen Robertson and Kirsten (SRK), the <br />design engineering consultants and construction quality <br />assurance consultants, inspected the main embankment. A <br />copy of the inspection report is attached. <br />Repair of a small section on the upstream face of the raise <br />was conducted October 26-29 under the direction of Dave <br />Hallman, the engineer-of-record for the project. A copy of <br />the repair report is also attached. <br />The inspection and excavation activities indicated that <br />ponded water on the crest of the embankment caused localized <br />cracking and erosion features. The crest road was recgraded <br />and is being maintained such that water will drain from the <br />road into the facility.
