A dexriptioa of the corrective action takes should be included with the DMR. 2. Reporting of Data Reporting of the data gathered in compliance with Part I.B.1 shall be oo a monthly bazis. Monitoring results shall be svmmarved for month and reported oa Division approved discharge monitoring report (DMR) forms (EPA form 3320- 1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the following month. If ao dixharge occurs during the reporting period, 'No Dixharge" shall be reported. The DMR Corms consist of four pages -the top 'original' copy, and three attached no~arbon-required copies.
