(Sept-Dec 94 )" <br />Regarding the missing Ap.°il 1993 monitoring, the records show SC- <br />3 was visited, as indicated in the 1993 Annual Hydrologic Report., <br />and ?[-10 and E-6 were not accessible due to rock slides. Copy of <br />the April 1993 groundwater monitoring record enclosed. <br />As the Division is aware, there Baas a personnel change at SMC in <br />May of '993. As a result of that change, the May 1993 monitoring <br />was not done; poor communication and misunderstanding of the <br />complicated monitoring requirements produced only a short list of <br />lab results for all of the 4th quarter samples taken on November <br />10, 1993, and the other missing values For 1993 noted above. <br />The monitoring parameters in effect during 1993 were developed <br />during Permit Revision Vo. 1 on March 12, 1991. Decision date <br />Marcie 27, 1991, effective date April 26, 1991, copy enclosed. <br />ACZ Laboratories, Inc., Steamboat Springs, Colorado was doing the <br />analysis for Somerset dining Company. They had been Provided <br />with the parameter list and should have provided all the required <br />results. Co;~ies of the chain of custody records for 1993 <br />monitoring are enclosed. <br />The monitorinr,• schedule in effect January to October 1993 was <br />from Permit Revision \o. 2, September 16, 1992. Decision date <br />September 25, :992, effective date October 16, 1992, copy <br />enclosed. This c;ianged to a schedule in effect from 1`ovember <br />7993 to April 199. by ':ethnical Revision ~o. 18, September 13, <br />1953, Decision date Xovcmber 10, 1993, effective date Xovembor <br />20, 1993, cop5• enclosed. <br />Tic Division was aware of any problems with the 1993 monitoring <br />prograrc. The inspection records show a review of the monitoring <br />rccorrls on '9arch 23 and June 30, 1993. Copies of relevant <br />portions of the inspection records enclosed. June monitoring was <br />done on June 30, July 1, and July 2, including monitoring of <br />Springs 1 -]1. July monitoring was done on July 28 and July 29. <br />Copies of the June and July, 1993 monitoring records enclosed. <br />4 <br />
