3.1.3 Regulatorv Submittal/Approval <br />NSI submitted routine logs, surveys, completion reports, Sundry Notices, monthly, quarterly, <br />and annual reports to appropriate agencies. In summary the following were submitted: <br />completed subsidence monitoring with temperature and CCL logs on 3M-TDR, 94-1 M, and 88- <br />1; submitted notifications for plugging and abandonment (P/A) of wells 91-1 V, 93-3V, and 4-2V; <br />changed status of 4A-4V and 4A-6V to shut-in; submitted Sundry Notice to convert 2A-2V to a <br />new water well; submitted Sundry Notice for 6H pull back. A major accomplishment of 2005 <br />was the submittal of the Revised Monitoring Plan to address UIC Permit modifications. <br />The conversion of 2A-2V to raw water well involved several perforation jobs and an acid <br />treatment. Insufficient flow for use as a water well was the result, and P/A will be performed in <br />2006. <br />The 6H pull back was completed as approved in the Mine Plan. A Sundry Notice was <br />submitted to modify 7H-21 drilling distance to approximately 1310 feet to avoid drilling or mining <br />into 1A cavity. The 7H-21 and 7H-1R wells were drilled; both wells were cemented and <br />pressure test data (7H-21) was sent to the EPA and BLM. The DHG Prospecting Bond was <br />released (P-1981-075). The storm water annual report was submitted <br />Approval was obtained to send up to one million gallons of high chloride water to the waste <br />pond from Nov.-Dec. One undesirable event, a spill contained on the road above the lined <br />pond, was reported in 2005 as a result of excess flow/pressure in the 12" pipe to process pond. <br />The BLM issued a letter of non-compliance for not performing corrective action in a timely <br />~- <br />
