Permit <br /> No. 83-33 , Annual Report <br /> FEDERAL TAX ID$' 84-0517799 <br /> Our files indicate that the anniversary date for this <br /> permit is June 23 . A Technical Revision was submitted on ???ad- <br /> dressing surface discharges from the site. We have a NPDES permit <br /> and SWMP for the site as required by the State Health Department. <br /> This pit HAS NOT been granted a Temporary Cessation and it HAS <br /> operated more than 180 days this year. The Financial Warranty for <br /> this operation is $75, 000, the permitted area is 125.88 ac. ± and <br /> the pit is divided into 7 Reclamation Phases. A sign that conforms <br /> with rule 6.7 Signs and Markers is at the entrance to the quarry on <br /> the scale house. Please refer to the Annual Report Map for the <br /> location of areas discussed in this report. This is a quarry <br /> operation, as a result we remove large amounts of material without <br /> disturbing large areas of land. <br />
