The report is in compliance with this rule. It was dated February 15, 2001, and received at the
Division on February 20, 2001.
Rule 2.04.13(1)(a): The Home and address of the permittee and permit number;
The report is in compliance with this rule. The name and address of the permittee and permit
number are Minrec, Inc., 2768 Compass Drive, Suite 101, Grand Junction, Colorado 81506, Permit
Number C-81-025.
Rule 2.04.13(1)(b): Location and number of acres disturbed during that year;
The report is in compliance with this rule. Less than 0.1 acre of land was disturbed during 2000 for
additional cover over Portal 1 structures and removal of two conveyor belt footers.
Rule 2.04.13(1)(c): Location and number of acres buc~lled and graded during that year;
The report is in compliance with [his rule. The disturbance area described under Rule 2.04.113(1)(b)
above was backfill, demolition and grading activities.
Rule 2.04.13(1)(d): Location and number noes topsoiled during that yenr;
The report is incompliance with this rule. The additional cover over the portal structures was best
material available. The grading of the areas of footer removal was with topsoil at the site.
