Due to <br />various factors, including the staff changes at Powderhorn, the report was not submitted until <br />January, 1999, approximately ] 0 months late (for the second straight year). I have also enclosed <br />a summary of AHR report requirements to assist the reviewing hydrologist. I have advised the <br />operator that such delays are not acceptable, and that enforcement action will likely be taken if the <br />1998 AHR is not submitted by the March 31 deadline. <br />I noted the following problems or discrepancies in my preliminary review. <br />The usefulness of the mine inflow map would appear to be enhanced if mining timeframes <br />were accurately depicted. The map submitted distinguishes only between "pre-1978 <br />mining" and mining conducted "1986 or previously". All areas mined since 1978 are <br />erroneously depicted as having been mined in 1986 or earlier. <br />2. One of the two upstream alluvial wells on Rapid Creek (GWMS-03a) was reported as <br />"destroyed by vandals" in September, 1997. I don't recall being notified of this, and the <br />monitoring plan specified in the PR-02 Findings, issued in December, 1997, specifies that <br />this well would be monitored May and September for water level, to monitor surface <br />subsidence impacts on creek flows in the southern permit area (along with other wells on <br />Rapid Creek and Cottonwood Creek). Operator will need to address the status of this <br />well and the need for repair or replacement. <br />Field parameters (temp., conductivity, and pH) were not included in the annual monitoring <br />data for LJTL piezometers, as specified in the permit, and the data will need to be <br />provided. <br />4. Required quarterly depth to water was reported for all refuse piezometers, with the <br />exception of missing second quarter data for Pz-307, for which problems in monitoring <br />
