' Third Perty Oversight 2 Weter, Waste & Lend, Inc. <br />San Luis Mine June 9. 1994 <br />' ^ August 23 -DMG gives approval for construction of the first 10 feet of the Phase <br />II, Raise I Embankment. <br />' ^ September 9 -Construction of the first 10 feet of the Raise I Embankment is <br />completed. <br />^ October 1 -The first DMG site inspection is conducted. <br />^ October 4 -BMRI resumes construction of the Raise I Embankment. <br />1 ^ October 8 -BMRI suspends construction on the Raise 1 Embankment until review <br />and conclusion of outstanding issues raised by the DMG. <br />^ October 28 -Site visit by DMG to initiate third-party review. <br />' ^ November 17 -BMRI resumes construction of the Raise I Embankment upon <br />approval from the DMG. <br />^ December 9 -BMRI completes construction of the Raise I Embankment. <br />^ May 16, 1994 -BMRI submits Final Construction Report to DMG. <br />1.3 WORK SCOPE <br />' Third-party technical oversight was conducted according to the tasks outlined in the <br />October 15 Agreement. These tasks are outlined below. <br />' 1.3.1 Task 1 -- Familiarization with Approved Plan <br />This work was started on October 25, and included an independent review of pertinent <br />design and permitting documents and correspondence concerning the tailings disposal facility <br />expansion. Documents provided by BMRI and DMG and reviewed by WWL are listed in <br />' Section 7 of this report. Key documents are cited in this report. A meeting with. BMRI, DMG, <br />and WWL was held at the mine site on October 28 to acquire additional technical clarification <br />on design and construction material issues. <br />1.3.2 Task 2 -- Field Construction Oversight <br />' A construction oversight schedule was established with BMRI and DMG. Field <br />construction oversight was conducted on November 11, 1993 and from November 17 to <br />' December 8, 1993. Oversight activities included observation of all phases of expansion <br />construction. Construction activity updates were provided in weekly reports to DMG by <br />WWL. DMG was to be given immediate notification about construction activities of concern; <br />however, no activities of concern were observed during construction. Recommendations <br />regarding daily or seasonal suspension of construction due to weather conditions were <br />reviewed. Construction of the Raise 1 Expansion was substantially completed ion December <br />9, 1993. <br /> <br />
