• Alan Cox <br />liomcstakc Mining Company <br />:\ugust 3, 2005 <br />E'agc 2 <br />Permit Issues <br />During the inspection, we observed that Cacility conditions arc accurately represented in the pemtit. <br />I'he Pitch Mine has been inactive for several years. Site activities are limited to site maintenance and <br />reclamation work. 'There is one wastewater treatment facility at the mine, <br />All the drainage ut the facility is directed to the Sediment Control Plant. This facility consists of a settling <br />pond, a clarifier, a horizontal pressure sand titter, and an outfall with a 4$-degree vee notch weir with an <br />ultrasonic Ievcl recorder. The settling pond and the outfall are currently active. The clarifier and sand <br />filter are not being used. The outfall appears is identified on the permit as 001. the plant and outfall are <br />located at 38° 23' S9" north, 106° 18' 30" west. <br />The Radium 'T'reatment Plant has been removed since our last inspection on August 25, 2000. The <br />pinnacle portal is still discharging, but that water drains to the Sediment Control Plant. <br />• tvuttrnrur [~rcr.yurarr,u: <br />:\ttached to this letter you will fmd a Customer Satisfaction Survey.
