• <br />1 <br />REPORT OF UNDESIRABLE EVENT <br />DATE/TIME OCCURRED: 4/20198 23:00 DATE /TIME REPORTED BLM: 4/21/98 1000 <br />LOCATION: NWl/4.NWl/4 QTR QTR, SEC. 1 T. 2S R.98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACII.ITY NUMBER: 1 f\-SHR OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-0l 18327 UNIT NAME/CA NUMBER: None <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />Oil/Saltwater Spill OillToxic Fluid Spill <br />Other (specify): Sodium Bicarbonate production fluid <br />CAUSE OF EVENT: Following the plant shut down and boil-out of April 1 I-17. flow to and from the <br />production wells for cavity #lA w;ts started and plua~in~ problems were noted On April ~0 an attempt to <br />start mining caused the infection water to pressured up in well l A-SHIt and water was forced to surface and <br />leaked around power supply cable onto pad. <br />VOLUMES OF POLLUTANTS: Discharged/consumed: Approximately 600 gallons <br />Recovered: Approximately d0 Qallons was drained to mud ~t <br />TIME REQUIRED TO CONTROL EVENT (in hours) 1/2 hour <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: Flow was reduced (1 1:30, 4-20-98) after plugging problem was noted and at 0900 on 4-21-98 <br />wash water was sent down the back side to remove blockade. A ditch was constructed to drain the standing <br />water. Minimal quantities of production water remained for clean-up. After the soil is dry, a loader will be <br />used to scrap up the bicarb contaminated soil for disposal in the mud pit. <br />CAUSE AND EXTENT OF PERSONNEL INJURY. None <br />OTHER FEDERAL, STATE ANLt LOCAL GOVERNMENT AGENCIES NOTIFIED: None <br />ACTION TAKEN TO PREVENT RECURRENCE: WRN will install a positive seal or packer around the <br />power supply cable.
