NW1/4 Q'TR QTR, SEC. 1 T. 2S R. 98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACILITY NUMBER: lA.-SHIt OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-0118327 UNIT NAME/CA NUMBER: None <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />OiVSaltwater Spill OiVToxic Fluid Spill <br />Other (specify): Sodium Bicarbonate production fluid <br />CAUSE OF EVENT: Dunne the plant shut down and boil-out excess volumes of plant water was produced <br />Operators were instructed to limit flow to process pond, and obtained approval to send 10.000-20.000 gallons <br />of barren liquor to the injection well 4A-3H. The monitoring wells (water level) were declinin and injection <br />was thought to be the most desirable method to divert excess quantities of production water. which can be <br />recovered during start-up. The operators failed to close'the valves to well 1 A-4HI and water returned uo <br />recovery well lA-SHR and spilled on~ad. <br />VOLUMES OF POLLUTANTS:Discharged/consumed: Approximately 20 gpm for -15 min. or 300 aIQ Ions <br />Recovered: None <br />T]ME REQUIl2ED TO CONTROL EVENT (in hours) 2 hours <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: The spill was not discovered until AM 4-13-98 and minimal quantities of production water was <br />available for clean-up. After the soil is drv, a loader will be used to scrap up the bicarb contaminated soil for <br />disposal in the mud pit. <br />CAUSE AND EXTENT OF PERSONNEL 1NNRY: None <br />OTHER FEDE STATE AND LOCAL GOVERNMENT AGENCIES NOTIFIED: None <br />ACTION TAKEN TO PREVENT .RECURRENCE: Management and operator s were infcrmed of the spill, in <br />the future valves to cavity #1 A will be closed. WRN will investieate and if possible install a positive seal <br /> <br />around the power supply cable.
