The name and permit number of the mine and the name and address of the permittee <br />are: <br />Carbon Junction Mine (CDMG permit number C-92-080) <br />Oakridge Energy, Inc. <br />4613 Jacksboro Highway <br />Wichita Falls, Texas 76301 telephone (940) 322-4772 <br />b). No additional acreage was disturbed in 2004. None of the maintenance activities <br />undertaken w 2004, including ditch cleaning, installation of flumes at the sediment pond <br />outfalls, berming topsoil.pile #2, and road maintenance, entailed additional disturbance. <br />c). No additional backfilling and final grading took place during 2004. North Pit <br />backfilling was estimated to be 60 percem complete as of the end of 2004. <br />Approximately 40 percent of the post-mine Carbon Junction Channel construction <br />remains to complete as of the end of 2004. <br />d). No additional topsoil replacement took place during 2004. <br />e). No additional revegetation, augmented seeding or cultural practices were undertaken <br />in 2004. <br />
