Best management practices (BMPs): schedules of activities, protu'bitions of practices, maintenance procedures, <br />and other management practices to prevent or reduce the pollution of waters of the State, BMPs also include <br />treatment, operating procedures, and practices to control site runoff, spillage or leaks, waste disposal, of drainage <br />from material storage, BMPs include stmctural and nonstructural controls. <br />T-- - , <br />2. Operator: the individual who has day-to-day supervision and control of activities occurring at the mine site. <br />3. Remediation, as used in this permit, means actions taken to reduce or eliminate the overall adverse environmental <br />impact associated with a pollutant source, The term includes actions such as re-routing of surface flows, re- <br />location or a pollutant source, Ore addition of pollutant neutralizing or immobilizing substances, capping and re- <br />vegetation <br />4. Significant Materials include but are not limited to: raw materials; fuels; materials such as metallic products; <br />hazardous substances designated under section 101(14) of CERL`LA; fertilizers; pesticides; and waste products <br />such as ashes, slag and sludge that Crave the potential to be released with stormwater discharge. <br />GENERAL MONITORING, SAMPLING, AND REPORTING REQUIItEMENTS <br />1. Routine Reoortin¢ ofData <br />Reporting of the data gathered in compliance with Part LB.1 shall be on a ovarterly basis. Reporting of all data gathered shall <br />comply with the requirements of Part I.E. (Genera] Requirements). Monitoring results shall be summarized for each calendar <br />quarter and reported on Division approved dischazge monitoring report (DMR) forms (EPA form 3320-1). One form shall be <br />mailed to the Water Quality Control Division, as indicated below, so that the DMR is received no later than the 28th day of the <br />monilr following the end of the quarter (foi exar~rle, the DMR for the first calendaz quarter must be received by the Division by <br />April 28th). If no discharge occurs during the reporting period, "No Discharge" shall be reported. <br />Tire DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-requited copies. After the DMR <br />form has been filled out and signed, the fora copies must be separated and distnbuted as follows: <br />The first orieinal signed copy of each dischazge monitoring report (DMR) shall be submitted to the Division at tbe following <br />address: <br />Colorado Department.ofPublic.Health and Environment '- <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />Additional copies aze for the perrrrinee records. An authorized person as identified in <br />Part I.E.6 shall sign them. <br />Calculations for al] ]imitations, which require the averaging of measurements, shall utilize an arithmetic mean unless otherwise <br />specified by the Division in the permit. ` <br />2.
