M-2003-030 <br />Prepazed: 8 Mar 2006 <br />nivisian of Minerals and GaologY Submitted: 7 May 2006 <br />$UI11I11aCV: <br />Glacier Peak Mining operated mechanized equipment on the Smoky Hawk and Elk Horn <br />claims in Teller County, CO to produce amazonite, smoky quartz, fluorite and other <br />related pegmatite minerals from 10 June 2005 through 9 July 2005. A John Deere 200 <br />track hoe was used for this operation. Follow-on mineral removal, prospecting, <br />reclamation, and claim upkeep continued on all Glacier Peak claims umil 19 October <br />2005. <br />Man and Table Dgpicting Ooerations• <br />Attached are maps and a summary table. The maps include an index map of ali Glacier <br />Peak claims (in yellow) and detail maps ofthe four claims covered under this permit. <br />The seven proposed production excavation sites approved under this permit are shown by <br />the letters A through G and are indicated with rectangular boxes. An orange area within <br />a rectangulaz box would-indicate an-openexcavation-where-work-will resume this season. <br />Presently, there are no open excavation sites. A rectangular box which is shaded green <br />shows a completed production excavation site and one which has been reclaimed. All <br />production excavation sites aze presently reclaimed. <br />The maps also include all the proposed and active exploratory prospect sites covered <br />under Colorado prospecting permit P-2003-014. The numbered blue "buttons" indicate <br />proposed areas. The red "buttons" enclosed within a 40' x 40' box aze active prospects. <br />Currently there aze two active prospects. Green shaded 40' x 40' boxes aze previous <br />prospects which have been opened and reclaimed. Reclaimed boxes may be less than 40' <br />x 40' because sample digs show whether or not there is mineralization and the full-size <br />prospect is not necessary. <br />The total areas proposed for excavation and mining under this mining permit remain less <br />than 3 acres which is the allowed acreage under this permit. To date, approximately .6 <br />acres have been disturbed and .8 acres have been reclaimed. <br />Current Disturbances: <br />An area of 0.25 acres total was disturbed during 2005 operations. Site B was opened <br />approximately 75% for a total of 0.20 acres. The remaining disturbances were generated <br />from completing one prospect and opening three new prospects, for a total of 0.05 acres. <br />This area, plus an additional, approximately 0.0.18 acre were reseeded and reclaimed. <br />
