(a) The name and address of the permittee and permit number: Hayden Gulch Terminal, Inc. 701 Market Street, Suite 700 St. Louis, MO 63101-1826 Permit No. C-80-003 (b) Location and number of acres disturbed during that year: There were zero (0.0) acres of disturbance at the Hayden Gulch Mine during 2005. (c) Location and number of acres bacl~lled and graded during that year: There were zero (0,0) acres backfilled and graded at the Hayden Gulch Mine during 2005. (d) Location and number of acres topsoiled during that year: There were zero (0.0) acres topsoiled at the Hayden Gulch Mine during 2005. (e) The species, location and number of acres of vegetation planted during that year, including any augmented seeding or cultural practices: There were zero (0.0) acres of vegetation planted at the Hayden Gulch Mine during 2005, (f) Location, number of acres and date of planting for all previously revegetated areas: The current permit area is spread between four pazcels. The locations of the pazcels are shown on Figure 1. The four parcels, and corresponding acreage, are as follows: • Parce1003 - 1.62 acres • Parcel HG-8 - 5.13 acres • Pazcel E - 1.03 acres • Slump Area - 1.14 acres Parcel 003 was seeded for herbaceous cover in 1987.. The seed mixture is presented in Exhibit 2 on Table 1. Pazcel HG-8 and Parcel E were seeded for herbaceous cover in 1982 and 1984/1985, respectively.
