Enclosed with this cover letter summarizing site activities during the <br />year ending September 29, 2005, are: (1) a signed DMG "ANNUAL FEE and <br />REPORT REQUEST" form; (2) a check for $688.00 for the annual fee due on or <br />before September 29, 2005; (3) a map depicting site conditions for the reporting <br />year; and (4) an air photograph reproduction at the same approximate scale as the <br />map showing July 2005 conditions for your reference. We have included both the <br />map and air photo in I 1"x17" format for convenience. We would be happy to <br />also provide the map in 22"x34" format if desired. <br />During the report yeaz mining activity occurred within Phases 7 and 8 as per <br />Exhibit D -Mining Plan Map in the permit, amendments and revisions, and as <br />shown on the attached Annual Report Map. Other site activities and <br />improvements included (1) completion of stripping and stockpiling of topsoil and <br />overburden within Phase 7; (2) completion of all grading and topsoil placement <br />around northerly Cell No. 1 (Phases 2, 3A, 4, 5 and 6) resulting in successful <br />release of these areas via Acreage Reduction 002 (AR-002) in December 2004; <br />(3) changes to dewatering configurations to continue dewatering for the mining <br />areas comprising Cell No. 2 in accordance with NPDES permit conditions. Phase <br />3B remains an active fresh/sedimentationpnnd azea. <br />Mining activities at the site occurred within the two phase areas noted above. <br />z329 wen Main sheet Disturbance consisted of the approximately 20.7 acres where mining was <br />Buite 20 l expanded compared to the prior reporting year.
