Vernon, M.D. <br />Executive Director <br />CERTIFIED MAIL N0: 149 900 178 <br />RE: Reported Violation for <br />Western Slope Carbon <br />CDPS Permit No. CO-0033146 <br />Gunnison County <br />For the Period(s) of: 01/01/85 - 03/81/85 <br />Dear Ms. Komatar: <br />The self-monitoring data you have provided on your Discharge Monitoring <br />Report(s) for the above referenced period(s) indicates you were in violation <br />of the discharge permit conditions indicated below: <br />Outfall Reported Permit <br />Number Parameter Result(s) Conditions <br />004 011 S grease, mg/1 40.2 10 max. <br />In addition, the frequency of analysis, sample type and number of exceptions <br />columns were not filled in. Please include this information with your reply. <br />It is imperative that the necessary steps be taken to maintain compliance with <br />permit conditions at all times. Please reply to this Division, in writing, <br />within fourteen (14} days after receipt of this letter as to what action and <br />measures you have or will be taking to abate this condition. Along with your <br />reply, you are to submit the individual results of all analyses performed for <br />those parameters listed above as being in violation during the referenced <br />period.
