All surface water stations were inspected and recorded daily (i.e.,
regularly scheduled work days) as per the permit terms and conditions. Inspection dates
were S/01-02, S/OS-09, 5/12-16, 5/19-22, and 5/27-29 as represented by the attached field
log report copies. These inspections and recordings identified readings at RS-1, RS-2,
RS-3 and RS-S as zero (0) inches; and RS-4 and RS-6 as dry. The monthly precipitation
accumulation for May was 0.25 inches, with the lazgest accumulation (i.e., 0.08")
occurring on May 20n'.
Vacuum on the tailing lysimeters was verified on May 28~, no water was present for
sampling and subsequent analysis as represented on the attached field log report copies.
Depths to water were collected for the tailing impoundment main embankment
piezometers on May 28a' and the West Pit and tailing embankment monitoring wells on
May 31s` as represented on the attached field log report copies. The May 28n' tailing
impoundment piezometer measurements identified these installations were either dry or
contained less than 12 inches of water as illustrated in Table 2. Tables 1 and 2 below
identify the depth to water measurements collected for each of the site monitoring wells,
piezometers and lysimeters.
The tailing impoundment collection pond underdrain flow was pumped intermittently
during the month to minimize the collection pond level. The leak detection system was
measured on May 31~`. The May collection pond underdrain flow average was 40.88
gpm and a total of 350 gallons of water was excavated from the leak detection

P.O.
