INTERNATIONAL URANIUM (USA) CORPORATION ~E C~/V~~ <br />8? ~?~ <br />Divi <br />RESPONSE TO MINERA JUNE 4, 2RA~M INSPECTION REPORT S/0°°t~y;°°raisendG~iO <br />8Y <br />MONOGRAM MINES <br />PERMIT # M-1978 222 UG <br />The Minerals Program Inspection .Report, based on a site inspection conducted June 4, <br />2001, notes problems to be corrected in association with Permit M-1978-222 UG. <br />Correction Dates of September 30, 2001, were stipulated in the Inspection Report. At the <br />request of the permittee, the Correction Date was subsequently extended to November 30, <br />2001. <br />1. Records and Reporting. ^~ _____ <br />Correspondence between the Division of Minerals and Geology and the permittee <br />were made in attempts to clarify the location of specific disturbance areas that are <br />to be reported under this Permit. In addition, the Permit files at the Division of <br />Minerals and Geology in the headquarters office in Denver were examined in an <br />attempt to locate relevant information on this Permit. The Permit files are <br />extremely limited, and no useful additional information was located. <br />A follow up field examination was conducted by the permittee on November 12. <br />A number of photos were taken of those sites identified on the 1" = 500' scale <br />map submitted herewith. The "Disturbed Areas" shown on the enclosed map are <br />the same as the "affected areas" identified in the November 20, 1995 map <br />submitted by R. A. Van Horn of Energy Fuels Nucleaz, Inc. (the prior permittee). <br />No file information has been located to support revision of the areas shown on the <br />Van Hom map. As noted in Mr. Bob Oswald's July 30, 2001, response to an <br />information request by International Uranium (USA) Corporation ("TUBA"), we <br />assume that the "affected areas" delineated on the Van Horn map comprise the <br />permit area. <br />No reclamation work has been conducted on any of the Disturbed Areas, as <br />designated on the map provided herewith, since the last work was performed by <br />the prior permittee(s). <br />The Monogram Mines Permit M-1998-222 UG includes a number of mine sites <br />and disturbance areas. All of the azeas on the Van Horn map were examined on <br />the recent field trip, except in some cases, the Disturbed Areas could not be <br />specifically identified due to the abundant natural vegetation and the lack of <br />cleazly discernible evidence of past disturbance. A description of each site is <br />provided herein; in addition, copies of photographs are included and have brief <br />descriptions with them. Reference is made to the 1' = 500" scale map for the <br />locations of each Disturbed Area; this map is on a topo base from the USGS Quad <br />
