If these data aze available provide them in revised <br />report pages. A notice of violation will be issued due to the above-referenced lack of <br />monitoring data. <br />16. The following required analytical data were not provided in Appendix [- Well Water <br />Quality Tables: <br />Annual water quality analyses were not performed for Well GP-3. Water <br />levels were obtained in May and June. No field parameters (including <br />depth to water, pH, temperature, and conductivity) were measured in the <br />third sampling period. <br />Annual water quality analyses were not performed for Well SOM-13. <br />Field parameters and annual water quality analyses were not performed for <br />Well SOM-45-H-2, Well SOM-C-76, Well 96-2-2, We1196-27-1, Well <br />RAV-4b, Well SOM-23-H-1, and Well SO.W-1 during the third sampling <br />period. Samples were collected for each of these wells during eazlier <br />sampling periods but still had required parameters missing from the data <br />presented in Appendix I. <br />Wells GB-l, B-32, and SOM-38-H-1 had damaged or bent casing and <br />were not sampled. <br />Please provide an explanation why these data, required according to the approved <br />monitoring plan, were not collected. If these data are available provide them in revised <br />report pages. As previously stated in item #S, the Division recommends that replacement <br />wells be installed at locations where wells have been damaged, so that the required <br />analytical data can be collected. A notice of violation will be issued due to the above- <br />referenced lack of monitoring data. <br />17. The values for total and dissolved iron are transposed on Table 1-31 for Well SOIvI-128- <br />H. Please check these data and submit a revised report page, if appropriate. <br />18. Please add a list of Maps to the Table of Contents at the beginning of the report. <br />
