<br />' 4.0 CONSTRUCTION QUALITY ASSURANCE AND QUALITY CONTROL (QA/QC) <br />4.1 General <br />SRK, the design firm, was retained by BMRI to provide QA/QC testing and full-time on-site <br />monitoring of the Contractor's activities. This QA/QC testing and monitoring wa;; required and <br />described in the original permit and permit amendment. <br />In addition, the owner implemented QA/QC monitoring procedures to further verify that the <br />t design objectives were met. The QA/QC adopted by the owner included visual inspection and <br />acceptance/rejection of all earthwork and monitoring all contractor safety procedures. These <br />' QA/QC procedures were outlined in the Technical Specifications presented in the Phase II Raise <br />2 Design Report. <br />Details of the QA/QC monitoring and testing results are described in the following sections. <br /> <br /> <br /> <br /> <br /> <br />1 <br /> <br /> <br /> <br /> <br />4.2 Earthwork <br />QA/QC testing of the raise fill resulted in no failing insi[u densities. All compaction tests met <br />or exceeded the minimum 85 percent of maximum dry density (ASTM D-698) specified. <br />Gradation tests indicated that the fill material has met the specification requirements. <br />A summary of all QA/QC testing for the raise fill is contained in Appendix A to this report. <br />4.3 Settlement Markers <br />Review of the settlement monitoring data indicated that the Bulk of the settlement occurred <br />during construction with no significant differential settlement. No signs of excess. ve settlement <br />of the tailings along the upstream toe of the raise, such as heaving or cracking, was observed <br />at any point during construction. <br />As of the October 17, 1995 survey the maximum settlement indicated by the readings has been <br />0.365 ft. Settlement marker locations and monitoring data are presented in Appendix B to this <br />report.
