MONITORING AND REPORTING
10. Sienatorv Requirements
a. All reports required for submittal shall be signed and certified for accuracy by the permittce in accordance with
the following criteria:
1) In the case of corporations, by • principal execartive pffiar of ai least the level of vice-president or his or
her duly authorimd repreemtative, if sncL represmtative is responsible for the Overall operation of the
facility from which the discharge described in the form originates;
2) In the case of a partnership, by a general partner:

b. Changes to autltoriaation. If m authoriation corder paragraph a) of this section is ao longer ~^~+A~a because
a different individual or position has responsibility for the Overall operation of the facility, a new authoriution
satisfying the tequirommts of paragraph a) of this section moat be submitted to the Division, prior to or together
with any reports, information, or applications tn be sigoed by m authorired representative.
c. Certification. Any person eiguing a document under paragraph a) of this section (except for the SWMP
certification) shell make the following certificatia~n:

FOOTNOTES -APPLICABLE TO PRECIDING PAGBS
a/ The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30)
consecutive-day period. Samples shall not be used for mom than one (1) reporting period.
b/ The seven ('7) day average shall be determined by the arithmetic mean of all samples takes in a seven (7) day period.
Samples may aot be used for more rhea one (1) reporting period.
c/ This limitation shall be determined by a single sample or set of samples ae required by Part I.B.3, Sample Type.
d! Monitoring is required only during periods of diac}iarge. If ao disclarge occurs, tins shall be reported at the specified
frequency. (See Part I.D.3.)
e/ TDS monitoring is Daly required for facilities discharging in the Colorado River Basin. TDS may be measured as
electrical conductivity when a satisfactory correJatioa with TDS has bxa established based upon a minimum of five
(5) samples. Sce the individual Certification Rationale for a determination as to whether monitoring is required.
f/ Phosphorus monitoring is required only for facilities whicL discharge into the following drainage basins: Cherry Creek
basin, Chatfield Reservoir downstream of the USGS gage at Waterton and on Plum Creep, Dillon Reservoir basin (i.e.,
• Tea Mile Creek, Snake River, Blue River, all tn'butaries to the Dillon Reservoir), and Bear Creek basis. See the
individual Certification Ratiomle for a derrrmination as to whether monitoring is required.
