Rule 3.1.7(7)(b) requires that, if ground water monitoring is required, the operator shall <br />include the following information as part of a permit application or permit modification to an <br />existing permit: a map that accurately locates all proposed ground water sample points and any <br />proposed points of compliance; the method of monitoring well completion where monitoring wells <br />are required; the method of sampling and frequency of sampling and reporting to the Division; the <br />pazameters to be analyzed, water quality analysis methods and QA/QC methods; the formations, <br />aquifers, or strata to be sampled; •the potential sources of ground water contamination that will be <br />monitored at each point of compliance; a time schedule for implementation; and ambient ground <br />water quality data sufficient to chazacterize potentially impacted Bound water quality. Further, <br />lowering of goundwater tables may impact or injure adjacent property owners, neazby wells, and <br />water rights in the vicinity of the quarry. Accordingly, the proposed condition should be imposed in <br />any 112 permit. <br />5. Proposed condition: <br />The Applicant shall do a baseline pre-blast survey of the facilities of the Black Hawk <br />- Central City Sanitation District Wastewater Treatment Plant and of the Silver <br />Dollaz Metropolitan District located within 200 feet of the affected land. Said <br />facilities shall be surveyed by Applicant in each month in which blasting occurs on <br />the site to check for damage. Blasting shall cease and a revised blasting plan shall be <br />resubmitted to the Division and to the Black Hawk- Central City Sanitation District <br />and the Silver Dollar Metropolitan District upon the occurrence of any damage to the <br />facilities. If a revised plan is approved by the Division, blasting may be resumed in <br />compliance with the revised plan and the facilities shall again be surveyed by <br />Applicant in each month in which blasting occurs. Upon the occurrence of any <br />additional damage to the facilities, blasting shall cease permanently. Prior to <br />initiating any blasting, the Applicant shall obtain and provide notarized ageements <br />between the Applicant and 1) the Black Hawk-Central City Sanitation District and 2) <br />the Silver Dollaz Metropolitan District and 3) the City of Black Hawk in which the <br />Applicant agees to provide full compensation for any damage caused by blasting to <br />the continuous, safe, and compliant operations of the respective entity's operations. <br />The term "damage" in said ageements shall be defined to explicitly include <br />structural damage to buildings; foundations; HVAC and odor control systems; <br />mechanical equipment (e.g., pumps, pipes, valves); wastewater treatment process <br />equipment (e.g., aeration basins, clarifiers, filters, bio-solids handling, and <br />disinfection); process monitoring equipment (sensors, cables, RTUs, and associated <br />supervisory control and data acquisition (SCADA) equipment); and highway lighting <br />and related power transmission and control systems. "Damage" shall also be defined <br />to include reimbursement for operational interruptions due tonon-structural impacts <br />to the wastewater treatment plant and highway lighting including, but not limited to, <br />SoftO <br />
