moving activities at the site and prior to any excavation exposing groundwater at the <br />site. The program shall contain all components specified in Construction Materials <br />_. ___..,.Rulp,_3;1.7(Z)(b). At a._minimum, the_srtonitoring.,prcagram_shall. include..four- .. <br />monitoring wells upgradient of the pit and four monitoring wells downgadient ofthe <br />pit drilled to the proposed final depth of excavation. An additional monitoring well <br />shall be drilled approximately mid-way along a line between the centroid of the pit <br />and the Black Hawk-Centra] City Sanitation District well. The nine monitoring wells <br />shall be drilled by the Applicant prior to any blasting or other earth-moving activities <br />and prior to any excavation exposing groundwater at the site. Applicant shall have a <br />geologist on site during 'the drilling of each of the monitoring wells who shall <br />describe the cuttings during the drilling of the monitoring wells in a written report for <br />each well, and the Applicant shall provide each report to the City of Black Hawk and <br />the Black I~awk-Central City Sanitation District along with a written summary of <br />how the wells were completed, including casing size, size and depth ofcasing slots, <br />and details of cementing and well development. The constituents to be sampled or <br />measured at each of the monitoring wells include: radioactive parameters (including <br />Boss alpha, radium 226 and radium 228, uranium, thorium 230 and 232), and <br />turbidity, and ground water levels. The monitoring wells shall be sampled monthly <br />and ground water levels shall be measured monthly for a minimum of four months <br />prior to initiation of any blasting or earthmoving activities at the site and prior to any <br />excavation exposing goundwater at the site. Monthly sampling and measurement of <br />the groundwater levels in the eight monitoring wells shall continue until completion <br />of the reclamation and final release of the reclamation bond. The monitoring results <br />shall be reported monthly to the Office of the State Engineer, Colorado Department <br />of Health and Environment, the City of Black Hawk Water Department, the City of <br />Central Water Department, and the Black Hawk-Central City Sanitation District. <br />Drill cuttings from the construction of the monitoring wells shall be monitored and <br />logged for radioactivity. In addition, the Applicant shall agree to conduct andpay for <br />monthly monitoring of the Black Hawk-Central City Sanitation District Wastewater <br />Treatment Plant well for all above-described pazameters, including groundwater <br />levels. If an exceedence of water quality standazds is documented in any sample of <br />surface or ground water that can or will leave the property boundaries, upon <br />becoming awaze of the test results, the Applicant shall immediately report the results <br />to the Colorado Department of Public Health and Enviromnent by telephone. The <br />Applicant shall resample the source for the parameter(s) in violation, requesting an <br />expedited turn azound time for the second sample results. Within five (5) working <br />days of the initial phone notification, the Applicant shall submit in writing to <br />CDPHE, with a copy to the Division, a copy of the sample results, and the following <br />information: a description of the water quality standards exceedence and what likely <br />caused it; the period of noncompliance with standazds, including dates and times <br />and/or the anticipated time when the dischazge will return to compliance; and all <br />steps being taken to reduce, eliminate, and prevent recurrence of the noncomplying <br />dischazge. <br />4of10 <br />
