Pursuant to Rule 1.4.2(3), DMG decision date for the application is <br />10/25/99. <br />DMG review of the application has identified the following adequacy issues. Please be <br />advised that the application may be found inadequate and may be denied on 10/25/99 <br />unless the following adequacy items are addressed to the satisfaction of DMG. Please <br />submit response to these adequacy issues to the DMG Durango Office, 701 Camino Del <br />Rio, Room 125, Durango, CO 81301, phone (970) 247-5469, FAX (970) 247-5104, by <br />10/20/99 to ensure sufficient time for DMG to verify compliance with the Act and Rules <br />and approve the application on 10/25/99. Should the applicant be unable to respond by <br />10/20199, please submit a written request for extension to DMG decision date prior to the <br />current 10/25/99 deadline. <br />ADEQUACY ISSUES <br />1. Proof of Notice DMG has received proof of application copy to the county Clerk <br />or Recorder, as required by Rule 1.6.2(1-Ic), and proof of mailing notice (Certified <br />mail whi receipt) to the Board of Supervisors of the local soil conservation district <br />and the Board of County Commissioners, as required by Rule 1.6.2(1-(al. However, <br />DMG records do not verify proof of mailing notice to all owners of record of surface <br />rights (BLM) of affected lands and lands within 200' of the permit boundary, as <br />required by Rule 1.6.2(1)(e).
