EXHIBIT R -Proof of Filing with County Clerk and Recorder (Rule 6.4.18): You must submit an affidavit <br />or receipt indicating the date on which the application was placed with the local County Clerk and Recorder. <br />NOTE: The legal description provided to the County Clerk and Recorder under Exhibit R (enclosed) states <br />that "The $yzantine Quarries 112 Operation affected land will be located in the southwest %< of Section 6 <br />and the northwest '/a of Section 7 Township 18 South, Range 71 West; and in the southeast % of Section 1 <br />and the northeast %a of Section 12 Township 18 South, Range 70 West, 6'" Principle Meridian." However, all <br />other portions of the application package submitted state and/or illustrate that the affected land will be <br />located in the southwest '/. of Section 6 and the northwest '/, of Section 7 Township 18 South, Range 70 <br />West; and in the southeast %< of Section 1 and the northeast %a of Section 12 Township 18 South, Range 71 <br />West, 6'" Principle Meridian. Please resolve this discrepancy in Exhibit R and gather the required affidavit <br />and signature with the corrected information. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to <br />begin our review. A decision date will be established 90 days from the date of receipt of all of the requested information. <br />Additionally, if you have already published notice you will need to republish notice, after the Division considers the <br />application submitted. This notice must be published once a week far four (4) consecutive weeks, starting within ten (10) <br />days of the date your application is considered submitted. We will notify you when you should initiate republication of <br />your notice. The final date for receiving comments is the 20th day after the Fourth publication or the next regulaz <br />business day. <br />You have sixty (60) days from the above date to submit all necessary documents that the Office needs for an application <br />to be considered filed. If, at the end of the sixty day period, the application has not been determined to be filed with the <br />Office, the Office shall deny the application and terminate the application fife. <br />This letter shall not be construed to mean that there aze no other technical deficiencies in your application.
