of 112 Construction Materials Reclamation Permit
Dear Mr. Clark:
On March 6, the Division of Reclamafion, Mining and Safety received yoar 112 Construction Materials Reclamation
Permit Application package for the Byzantine Quarry, M-2007-014. Preliminary review of the information received
determined that the following items must be received before the Division can consider your application as being
submitted and technical review can begin:
APPLICATION FORM:
The Division requires the company name listed throughout the application to match the company name listed
with the Colorado Secretary of State (as officially appears "Larsen Colorado, L.L.C."). This punctuation (comma
and periods) is required in all places the company name is mentioned. Please correct all portions of the original
application (enclosed) and return it. You may close to insert the proper punctuation onto the enclosed
application, rather than filling out a new application. However, all places where the company name is mentioned
must appeaz as "Lazsen Colorado, L.L.C." exactly.
In addition, on page 8 of the (enclosed) application the portion of the page that has been notarized by Andrea
Lambert must also reflect the official company name of "Larsen Colorado, L.L.C.". Please make these required
corrections and return the application.
EXHIBIT A - Leeal Description [Rule 6.4.1): The legal description must identify the affected land and be
wholly adequate for field location of the property. It can be in the form of metes and bounds survey or a
description by reference to township, range and section to at least the neazest quarter quarter section. Where
applicable, the street addressor lot numbers maybe used.
NOTE: The legal description provided under Exhibit A (enclosed) states that "The Byzantine Quarries 112
Opem6on affected land will be located in the southwest %a of Section 6 and the northwest %a of Section 7
Township 18 South, Range 71 West; and in the southeast %< of Section 1 and the northeast '/< of Section 12
Township 18 South, Range 70 West, ti's Principle Meridian." However, all other portions of the application
package submitted state and/or illustrate that the affected land will be located in the southwest'/<of Section 6
and the northwest % of Section 7 Township 18 South, Range 70 West; and in the southeast'/< of Section 1
and the northeast '/a of Section 12 Township I $ South, Range 71 West, ti`s Principle Meridian. Please resolve
this discrepancy in Exhibit A.
