Walther <br />, <br />Henderson <br />CG 80640 Erecmive Director <br />, Michael g Long <br /> D~mvon D~reaor <br />RE: Adequacy Review - Lohmann Sand and Cravel one -File No. Af--2001-017 <br />Dear Ms. Mayer: <br />Listed below are the Division's adequacy review comments on the Lohmann 112 construction materials <br />permit application. <br />On page 11 of the application, the applicant stated that the design information, technical specifications, CQA <br />procedures and a sample construction report for the proposed slurry wall would be submitted during the DMG <br />112 review process. The Division anticipates that the same information will be submitted for the proposed <br />clay liner. Since this information has not been received, adequacy comments on the slurry wall, clay liner and <br />related reclamation costs cannot be included in this review. <br />The current decision due date for this application is June 18, 2001. Please be advised that your application <br />may be deemed inadequate and may be denied on June 18, 2001, unless the following adequacy review items <br />are addressed, along with any adequacy review items that arise from the yet to be submitted slurry wall and <br />clay liner information. <br />If additional time is needed to prepare and submit an adequacy response, the applicant may waive their right <br />to a decision within 90 days and request an extension of time. However, such a request must be made in <br />writing and received by the Division on or before June 18, 2001. <br />The following items need to be addressed as part of this adequacy review: <br />1) Rule 6.2(2) requires that "Maps, except the index map, must conform to the following <br />criteria: <br />bJ must be prepared and signed by a registered land s:~rveyor, professionnl <br />engineer or other qualifred person ". <br />Please provide the Division with a set of maps with the appropriate signatures. <br />2) On page 10 of the application, Exhibit E-Reclamation Plan, it is stated that imported inert fill <br />will be used as part of the reclamation. <br />Please provide the Division with the information required under Rule 3.1.5(9) (a) through (f), <br />concerning the backfilling of structural fill material generated outside of the approved permit <br />area. <br />
