SEI/4 QTR QTR, SEC. 26 T. /S R 98W 6TH Meridian <br />County: Rio Blanco State: Colorado <br />WELL/FACILITY NUMBER: SH well OPERATOR: White River Nahcolite <br />SURFACE OWNERSHIP: Federal Indian Fee State <br />LEASE NUMBER: C-0118327 UNIT NAME/CA NUMBER: <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />OiUSaltwater Spill Oilri'oxic Fluid Spill <br />Other (specifyLSodiumBkarbonate Liquor>90.000 PPM <br />CAUSE OF EVENT: Production well SH was not back flushed after what was going to be a temporary shut <br />down, which turned into approx. two day outage to repair centrifuge motor control. When recovery wells for <br />1 A and SH were turned-on, liquor from 1 A flowed to SH and down to blockage and backpressure caused spray <br />to exit around wellhead rubber seal and out power supply cable. <br />VOLUMES OF POLLUTANTS: Discharged/consumed: Approximately 200 gallons <br />Recovered: Ogallons <br />TIME REQUIRED TO CONTROL EVENT (in hours] Two L) hours for detection and control <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE; CLEAN UP PROCEDURES <br />AND DATES: As soon as the leak was detected, the valve between SH and lA was closed. Some of the <br />liquid flowed to the mud pit, the rest was contained within forty feet of the well head. All the liquid was <br />readily absorbed i[rto the soil and not recoverable. Ditch to divert spills to mud pit was improved. After the <br />soil dries out next Spring the surface will be scraped to remove bicarb. <br />CAUSE AND EXTENT OF PERSONNEL INJURY: None <br />OTHER FEDERAL, STATE AND LOCAL GOVERNMENT AGENCIES NOTIFIED: Colorado Division <br />of Minerals and Geology <br />ACTION TAKEN TO PREVENT RECURRENCE: Recovery wells will be properly back flushed with wash <br />water to maintain flow and extra attemion will be required during start-up to monitor flows. <br />GENERAL REMARKS: <br />RECEIVED BY: <br />(name)/ (title) <br />Cd ~COti <br />Ed Baker <br />WRN EHS Manager <br />Date: <br />
