The design of the over- <br />flowhose from the #1 to #2 tank was not sufficient for the flow into the #1 tank from the process pond pump. <br />As a result, the #1 tank over flowed onto the ground. <br />VOLUMES OF POLLUTANTS: Dischazged/consumed: Approximately 250 ¢allons <br />Recovered; O~ailons <br />TIME REQUIRED TO CONTROL EVENT (in hours) One (1) bows for detection and control <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES <br />AND DATES: As soon as the leak was detected, the pump was turned off. Following the spill, the pump was <br />operated intermittently at lower rate to allow flow to #2 and #3 tanks. The coarse subsoil absorbed all the <br />liquid acrd none was recoverable. The spill flowed down the ea~banicment to the pond access road <br />approximately 50 feet. The spill was contained within the pond fence area. After the soi! dries out next spring <br />the surface will be scraped to remove bicazb, and azea re-seeded until cover is established. <br />CAUSE AND EXTENT OF PERSONNEL INJURY: None <br />OTHER FEDERAL, STATE AND LOCAL GOVERNMENT AGENCIES NOTIFIED: Colorado Division <br />of Minerals and Geoloey <br />ACTION TAKEN TO PREVENT REC[TRRENCE: Operations were notified to maintain proper flows to <br />eliminate spills.
