Sample forms to docurr.training activities aze <br />included. <br />Crew supervisors should be responsible for instructing those under their direction to implement <br />the S WMP so the conditions of the Stormwater Dischazge Permit aze achieved. <br />4.6 Testing Tor Non-Stormwater Discharges <br />This site was evaluated for the presence ofnon-Stormwater discharges during May of 1996. The <br />procedure used to evaluate the area for the presence of non-Stormwater discharges consisted of <br />dry weather observations of the site. At that time there were no buildings or structures associated <br />with the potential mining operation at the site. There are no mine adits or shafts known at this <br />site, or associated with this operation, therefore, there is no mine drainage. <br />Naturally occurring springs have been identified at this site. There are no spoils piles at this site. <br />No sanitary facilities associated with the mining operation aze currently located here. There <br />currently are no dewatering discharges at this site and no structural conveyance systems for the <br />Stormwater dischazges. Potential discharge points observed included the perimeter of the site. <br />If any unauthorized sources ofnon-Stormwater dischazges are noticed at the site, the S WMP <br />Administrator shall be notified immediately. The source and condition of the discharge will be <br />investigated. If the discharge is perceived to be a potential contaminant source to Stormwater or <br />permitted process water (groundwater dewatering), the Administrator or designated personnel <br />shall notify the Colorado Department of Public Health and Environment. <br />SWMP CERTIFICATION <br />I certify under penalty of law that a complete Stormwater Management Plan, in compliance with <br />Part I.C.2 of the Sand and Gravel Mining general permit has been prepared for my facility. <br />Based on my inquiry of the person or persons who manage the system, or those persons directly <br />responsible for gathering the information, the Stormwater Management Plan is, to the best of my <br />knowledge and belief, true, accurate, and complete, as written. I am awaze that there are <br />significant penalties for falsely certifying the completion of said S WMP, including the <br />possibility of fine and imprisonment. <br />Operator/SWMP Administrator <br />Date <br />cc: File #96-133 <br />
