<br />6. Rule 6.3.6 Exhibit F- List Other Permits and Licenses Required The applicant states that no other <br />permits are required. The applicant may need to apply for a County Special Use Permit in Boulder <br />County. You should contact the County to determine if a special use permit is needed. <br />Rule 6.3.12 Exhibit L- Permanent Man-Made Structures A road is depicted on the maps that bisects <br />the North Quarry. The definition of"Structure, Significant, Valuable and Permanent Man-Made" <br />includes maintained or public roads. Please inform the Division as to the owner of the road. In <br />addition, where the mining operation will adversely affect the stability of any significant valuable or <br />permanent man-made structure located within two hundred (200) feet of affected land, the Applicant <br />may either; <br />(a) provide a notarized agreement between the Applicant and the Person(s) having an interest in the <br />swcture, that the Applicant is to provide compensation for any damage to the structure; or <br />(b) where such an agreement cannot be reached, the Applicant shall provide an appropriate <br />engineering evaluation that demonstrates that such structure shall not be damaged by activities <br />occurring at the mining operation (including the placement of fill). <br />8. Attached, please find a copy of a letter from the Office of the State Engineer regarding the proposed <br />operation. <br />Please be advised that the Lykins Gulch application may be deemed inadequate, and the application may <br />be denied on December 11, 2000 unless the above mentioned adequacy review items are addressed to the <br />satisfaction of the Division. [f you feel more time is needed to complete your reply, the Division can grant <br />an extension to the decision date. This will be done upon receipt of a written waiver of your right to a <br />decision by December I I, 2000 and request for additional time.
