6.3.6 EXIIIBIT F -List of Other Permits and Licenses Required <br />Please review this exhibit to determine if there are any other permits or licenses besides <br />those indicated that may be required to conduct this mining operation. If other agencies <br />have been contacted, please list those here. Enclosed for your review are any comments <br />that we have received from other agencies that DMG solicits for comment on new <br />applications. <br />6.3.7 EXHIBIT G -Source of Legal Right-to-Enter <br />It is permissible to allow a signed statement by the landowner that the applicant has the <br />legal right to enter and mine, however, the signed statement must be acknowledged by <br />a Notary Public. Please provide a signed and notarized statement. <br />6.3.8 EXHIBIT H -Municipalities Within aTwo-mile Radius <br />No comment. <br />6.3.9 EXHIBIT I -Proof of Filing with County Clerk <br />Please provide an affidavit or receipt indicating the date on which the application was <br />placed with the local County Clerk and Recorder for public review. <br />6.3.10 EXIIIBIT J -Proof of Mailing of Notices to Board of County Commissioners <br />and Soil Conservation District <br />This section is adequate as submitted. <br />6.3.12 EXHIBIT L -Permanent Man-Made Structures <br />Please identify the owner(s) of the livestock fence and if necessary provide a notarized <br />agreement between the applicant and the owner of the fence to provide compensation for <br />and damage that may occur to the fence. If you aze the owners of the fence, simply <br />provide a statement indicating that to be the case. <br />This concludes the Division's preliminary adequacy review of this application. Please <br />remember that the decision date for this application is May 12, 2005. As previously <br />mentioned if you aze unable to provide satisfactory responses to any inadequacies prior to <br />this date, it will be your responsibility to request an extension of time to allow for <br />continued review of this application. If there are still unresolved issues when the <br />decision date arrives and no extension has been requested, the application will be denied. <br />
