Index map- The county map and the map drawn on graph paper are adequate <br />to locate the site, however, the prepazed map should also be titled, include a <br />scale and show names of all land owners within 200 feet of the affected area. <br />Mine Plan Map- This map must be titled, include the owners of land within <br />200 feet of the affected area, identify owner of substance to be mined and <br />identify owners of structures within 200 feet of affected area boundaries. In <br />addition to the features already included (topsoil and product stockpiles) the <br />map should also include stream channels, impoundments and diversions and <br />indicate the direction that material extraction will proceed. <br />Reclamation Plan Map- This map must be titled and show the gradient of all <br />reclaimed slopes. In addition, it must state the thickness of replaced <br />overburden and topsoil. <br />Please remember to include dimensions of the proposed mining area as well as <br />an appropriate scale on all maps. <br />6.3.6 EXHIBIT F -List of Other Permits and Licenses Required <br />Enclosed for your review are any comments that we have received from other agencies <br />that DMG solicits for comment on new applications. One agency that you will need to <br />contact is The Water Quality Control Division of the Colorado Department of Health and <br />Envirorunent to determine whether or not you will be required to obtain storm water <br />permits for your site.
