• <br />EXHIBIT "A" <br />SPECIAL CONDITIONS <br />1. This permit is issued to widen and upgrade an existing agricultural <br />field access. <br />2. This permit is only for use and purpose stated in the application and <br />permit. Any changes in the type and use and/or volume of traffic using <br />the access may require the submittal of an Application for Access Permit <br />Eor review. <br />3. The left turn movements in and out of [tie driveway may be prohibited a[ <br />some future date. <br />4. Contractor shall follow [he applicable construction specifications set <br />Forth Icy the Department of Highways in the latest manual "Standard <br />Snecifica[ions for Road and Bridze Construction". <br />5. Driveway shall Ise constructed to 35 feet in width, with a 40 foot radii, <br />and at a 90° angle [o the highway. <br />6. The firs[ 20 feet beyond the closest highvay lane, shall slope down end <br />away from the highway at a 2x grade to ensure proper drainage control. <br />7. The new pavement shall slope on [he same plane as the present pavement <br />surface. <br />8. The surfacing in the widening and the access approach to [he right of <br />way lines shall be 4 inches of Ho[ Bituminous Pavement (Grading E) and 4 <br />inches of 3/4 inch gravel. <br />9. Compaction of subgrade, embankments and backfill shall comply with <br />Section 203.11 of the Divisions' standard specifications. <br />10. Compaction of Hot Bituminous Pavement shall comply with Section 401.17. <br />11. If frost is present in Che subgrade, no surfacing material shall be <br />placed until all frost is gone or removed. <br />12. Sav asphalt to assure a straight edge for patching. <br />13. Fill/cut slopes shall match existing slope on the roadway and at 6:1 on <br />[he approach. <br />14. Owner is responsible for any utilities disrupted by the construction of <br />this driveway and all expenses incurred for repair. <br />15. Any damage to any existing highvay facilities shall be repaired prior to <br />continuing other work. <br />16. All existing drainage structures shall be extended to accommodate all <br />new construction and safety standards. <br />17. A nev 18 inch corrugated metal pipe of like material shall be used Co <br />extend the existing culvert. The culvert extension shall be joined <br />using a standard bond. <br />18. Any additional permits required by other Federal, State or local <br />government agencies or ditch companies are the responsibility of the <br />applicant and property owner. <br />19. _ Gork shall BECIN AFTER 8:30 A.M. and all equipment shall be off the <br />roadway BEFORE 5:00 P.H. each day.
