Please address. <br />EXHH3IT G -WATER INFORMATION <br />14. The applicant has proposed to monitor the 2" PVC wells quarterly to establish baseline data <br />prior to mining. The Division encourages the operator to monitor the wells on a monthly basis <br />at least until 12 months of baseline data have been collected, prior to monitoring at a frequency <br />of quarterly. Please comment. <br />EXHIBIT H -WILDLIFE INFORMATION <br />15. Prior to Division or Boazd approval the application will need to be reviewed and commented <br />upon by the State of Colorado Division of Wildlife (CDOW). Please provide a CDOW Wildlife <br />Impact Statement for the proposed "Milliken Site." <br />EXHIBIT L -RECLAMATION COST <br />Exhibit L, Reclamation Cost will need to be revised as follows: <br />Item H. 20% slurry wall installation cost should be based on an average depth of 48' (45' sand & <br />gravel + 3' key depth into shale) rather than 45' <br />Item H. 20% sluny wall installation cost should be based on a unit cost of $3.00 rather than <br />$2.50/sq.ft. (This cost is based on the Division cost estimating experience with a variety of <br />other slurry wall installations). <br />Please provide an itemized cost for pit dewatering at the time of maximum allowable <br />disturbance (this cost is necessary in the event of a bond forfeiture and the Division then <br />becomes responsible for hiring an independent contractor to complete the reclamation plan). <br />Item I. Equipment mobilization demobilization cost will need to include the type and number of <br />pieces of equipment used to reclaim the site during a maximum disturbance scenario, ie: <br />equipment used for backfill & grading, topsoil reapplication, revegetation, etc. <br />EXHIBIT S -PERMANENT MAN-MADE STRUCTURES <br />19.
