(b) A map or description which shall: <br />• (i) Clearly show or describe towns, rivers, streants, or other bodies of water, local <br />landmarks, and any other information, including routes, streets, or roads and accurate distance measurcmcnts <br />necessary to allow local residents to readily identify theproposed permit area; <br />(ii) Clearly show or describe the exact location and boundaries of the proposed permit <br />area; <br />(iii) State [he namc of the U.S. Geological Survey 7.5-minute quadranele map(s) <br />which contain(s) [Ite area shown or described; and <br />(iv) -f a map is used, indicate [he north point. <br />2.07.3(4): <br />(c) The location where a copy of the application is available for public inspection under <br />(d) The name and address of the Division to which written comments, objections, or requests <br />for informal conferences on the application may be submitted; <br />(e) If an applicant seeks a permit in which the affected area would be within 100 fee[, <br />measured horizontally, of the outside right-of-way of a public road except where [nine access roads or haul roads <br />loin such right-of--way line, the notice shall include a concise statement describing that particular portion of the road <br />which is located within 100 fee[, treasured horizontally, of the affected area. The notice shall also includc a <br />statement indicative that a public hearing, for the purpose of determining whether the interests of the public and <br />affected landowners will be protected may be requested by contacting the Division in writing within 30 days after <br />[he last publication of the notice. The notice shall also state. that any such hearing would be held in the. locality of <br />the proposed mining operation; and <br />If an applicant seeks a permit itrvolvbrg relocation or closure of a public road, a concise <br />statement identifying the road, describing the portion [o be relocated or closed, the route of the proposed relocation <br />and the duration and timing of the relocation or closure. The notice shall also include a statement indicating that a <br />public hearing, for [he purpose of detemrining whether dte interests of the public and affected landowners will be <br />protected may be requested by contacting the Division in writing within 30 days after the last publication of the <br />notice. The notice shall also state that any such hearing would be held in the locality of the proposed mining <br />operation; and <br />(g) If the application includes a request for an experimental practice, a <br />statement indicatin¢ than an experimental practice is requested and identifying the provisions of those rules for <br />which a variance is requested. <br />(3) (a) Upon receiut of a complete application for a permit, a permit revision, <br />technical revision, or permit renewal, the Division shall issue written notification of <br />(i) The applicant's intention to conduct surface coal minim and <br />reclamation operations on a legally described tract of land: <br />(ii) The application number; <br />(iii) Where a copy of the application may he inspected. <br />(iv) Where comments on the application may be submitted. <br />Permit Renewal No.3 2.07-2 3/5/99 <br />
