(b) A map or description which shall:
• (i) Clearly show or describe towns, rivers, streants, or other bodies of water, local
landmarks, and any other information, including routes, streets, or roads and accurate distance measurcmcnts
necessary to allow local residents to readily identify theproposed permit area;
(ii) Clearly show or describe the exact location and boundaries of the proposed permit
area;
(iii) State [he namc of the U.S. Geological Survey 7.5-minute quadranele map(s)
which contain(s) [Ite area shown or described; and
(iv) -f a map is used, indicate [he north point.

(c) The location where a copy of the application is available for public inspection under

statement identifying the road, describing the portion [o be relocated or closed, the route of the proposed relocation
and the duration and timing of the relocation or closure. The notice shall also include a statement indicating that a
public hearing, for [he purpose of detemrining whether dte interests of the public and affected landowners will be
protected may be requested by contacting the Division in writing within 30 days after the last publication of the
notice. The notice shall also state that any such hearing would be held in the locality of the proposed mining
operation; and
(g) If the application includes a request for an experimental practice, a
statement indicatin¢ than an experimental practice is requested and identifying the provisions of those rules for
which a variance is requested.
(3) (a) Upon receiut of a complete application for a permit, a permit revision,
technical revision, or permit renewal, the Division shall issue written notification of
(i) The applicant's intention to conduct surface coal minim and
reclamation operations on a legally described tract of land:
(ii) The application number;
(iii) Where a copy of the application may he inspected.
(iv) Where comments on the application may be submitted.
