DEFICIENCY NOTICE CMLRB 112 Permit Applicelion <br />Agile Slone Systems, Inc. <br />30. The Division learned at the pre-operation inspection that the intent of the <br />application was to include Gravel Expansion Blocks I and 2 and Granite Quarry <br />Expansion Blocks I and 2 in the application as land to be affected and that these <br />areas aze phases of the total mining operation. It was stated that it was not the <br />intent to bring these areas into the permit by amendment at a later date as <br />previously understood by the Division. If these areas are part of the application <br />affected land as phases of the mining operation, provide appropriate discussion in <br />all appropriate exhibits and change the Mining Plan Maps to show a phased <br />mining operation. If these mining areas aze not to be a part of the present <br />application, remove them from the maps to avoid confusion. If they aze to be <br />added at a later date, an amendment could be filed at that time. <br />As has been previously stated, all phases or expansions are included in the <br />application. All maps and discussions and exhibits have been thoroughly <br />reviewed and revised to clarify this conclusion or expectation. <br />31. During the pre-operation inspection it was learned that the location of the south <br />overburden and topsoil stockpiles will be changed. Please provide a discussion of <br />the new locations in the mining plan and show them on the Mining Plan Map. <br />The topsoil stockpile location has not been changed. While the visual <br />impairment permanent berm locations have not changed, their configuration <br />has been enhanced for more effective screening to satisfy expectations noted <br />in the applicant's County CUP. They are readily shown on the Mining Plan <br />Map and discussed in Exhibits D, E and H. <br />32. Show the power line that traverses the permit area to the ranch headquarters on <br />the Pre-Mining Plan Map. List the power line and the owner in the appropriate <br />exhibits. If an easement is associated with the line, show it on the map and list <br />the owner. <br />The poles for the power line that traverses the permit area to the much <br />headquarters on the Pre Mining Plan map are shown. The ownership is <br />denoted in the legend with the pole symbol and the "West Plains Energy" <br />label next to it. The power line and owner are noted in exhibits D and S. <br />The easement only exists for the poles and air, not the ground under the <br />lines. <br />33. Submit data concerning the effects of blasting operations on structures within <br />5000 feet. Provide change type and weights and scaled distances to structures. <br />See Vibra-Tech Blasting report and Pre-Blast Inspection Reports in the <br />13 <br />
