a
• 1. Operator shall comply with applicable requirements of surface management
agency, 30 CFR 315. I ~, or approved State program.
• MCC has reviewed, and will comply with, all applicable requirements of
30 CFR 815.1 and the Colorado Mined Land Reclamation Regulations
stated in C.R.S. 34-33-101.
2. Install casing and blowout preventers in drillholes in known oil, gas, and
geothermal azeas.
• The azea neaz the North Fork River is not in a producing natural gas field.
Minor gas has been encountered in some of the previous drilling efforts in
the area. The proposed drilling will penetrate the Rollins Sandstone,
therefore blowout preventers will be used. Casing will be set in each hole
as necessary to maintain circulation of cuttings to the surface.
3. Drill hole plugging requirements.
• Each drill hole will be plugged according to the requirements outlined in
this section: The hole will be plugged with cement from 50 feet below to
30 feet above any coal bed over 4 feet chick and any aquifer.
• The remainder of the hole will be filled with a completion mud (bentonite
• gel) and drill cuttings or cement to within ~ feet of the surface. A cement
surface plug will be set, capped with an appropriately marked brass or
aluminum monument, at or below ground level.
4. Retention of drill logs by applicant.
• MCC will retain in its files all drill logs and will make them available for
inspection by any authorized officer. MCC may conduct geophysical
logging and/ or collect core from this project.
5. Utilizing drill holes as water monitoring wells.
• MCC does not anticipate encountering any significant aquifers during this
drilling program. However, MCC does intend to complete this hole as
water monitoring well.
6. Maps of Project Area
• A map of the proposed project area at a scale of 1" = 400', showing all
borehole locations, mine mapping, existing roads, pads, and contours are
attached.
7. Surface Owners and Subsurface Owners of Record
